KU Pharmacy Replacement Name Tag Request

Your name tag signifies you are a student at the KU School of Pharmacy. You will use it throughout your four-year tenure at School of Pharmacy when participating in the required pharmacy practice observation experiences of some courses; rotation and/or observation experiences at KU Student Health Center facilities; KU Medical Center-Kansas City; the KU School of Medicine/Pharmacy-Wichita; when participating in official School of Pharmacy community service projects/events; and, other activities associated with the KU School of Pharmacy where the wearing of the name tag is considered appropriate and professional.

There is a $15.00 charge for replacement name tags. Requests will not be processed until payment received. If you leave this form without completing the $15 payment, you will need to complete and submit the form again.
