Residency Questionnaire

Applies to: Recently applied Lawrence and Edwards Campus students

Purpose: To gather additional information about a student's residency status to determine eligibility for residency for tuition purposes.

Frequency: This form should only be submitted once to the University Registrar within 30 days of the notification request from the Office of the University, typically at the time of admission. This form may not be used in lieu of a residency application after the first term a student is enrolled. Completing the Residency Questionnaire does not automatically qualify a student for resident tuition rates. The University Registrar’s Office may request additional information or may require a different form to be completed in order to determine your residency status. Please continue to monitor your emails for additional information requests.

Kansas Residency

List the day you most recently moved to Kansas. If your current address is not in Kansas, when will you be moving to Kansas?

60 Month Rule

If you've previously lived in Kansas, left the state, and moved or plan to move back to Kansas within 60 months (5 years), you may qualify for residency under the 60 Month Rule.

Please provide the dates below of when you previously lived in Kansas.
If you have lived outside of Kansas, provide the information below of when and where you lived prior to moving to Kansas.
Please provide documentation to verify when you last lived in Kansas as a bona fide resident and when you returned to live in Kansas as a PDF, JPEG, or PNG file. Examples: Kansas tax return, high school or college transcripts, lease, utility bills, etc.

Parent/Guardian Information

Please provide the dates of your parent/guardian's previous physical residence in Kansas if applicable.

If your parent/guardian previously lived in Kansas, provide the start and end date of their physical residency in the state.
If you are a dependent for tax purposes, and selected yes to having a divorced parent living in the state of Kansas, you may qualify for residency under the Divorced Parents Clause. Please provide the first page of their Kansas tax return. Do not include their federal taxes.

Education

If you've been continuously enrolled as a full time college student, enter the date you started attending as a full time student. Or, enter the date when you will start as a full time student.

Financial Support

Complete the Financial Support spreadsheet to list all financial support for the past twelve months. Include scholarships, loans, grants, employment, personal savings, interest, governmental benefits, monetary gifts, spousal contribution, etc. Please save the spreadsheet as a PDF before attaching to this form.
